Please fill out the new patient paperwork sent to your email. If you cannot complete them, please arrive 15 minutes before your appointment to complete them in the office.

Please bring any MRIs/X-Rays that you have to your appointment (even if they are old).

We encourage you to bring some additional ears with you, be it your significant other or a loved one. We don’t want you to miss any important information about your condition.
